Seizure treatment with Rimegepant, also known as Vydura

Vydura can be used both preventively and as a seizure treatment for migraine.

The preparation

Vydura has been developed specifically for the treatment of migraines. Vydura can be used as a seizure treatment but is also approved as a preventative for adults with episodic migraine who have at least 4 migraine attacks per month.

Vydura is not thought to cause overuse headaches and can therefore be used as a seizure treatment beyond the recommended 9 days with triptans or other pain relievers, for example.

Mechanism of action

It is known that CGRP (calcitonin gene-related peptide), which is formed in our nervous system, plays an active role in a migraine attack.

Rimegepant, which is the active substance in Vydura, is thought to work by binding specifically to the receptor of the CGRP molecule, thereby preventing the forwarding of pain signals to the brain. This can decrease the number and strength of your migraine attacks.

Vydura has a half-life of about 11 hours, which means it is out of the body after a few days.

Side effects

About 1-10% experience nausea. Very rarely (about 0.1%), rashes and difficulty breathing are observed.

If you experience side effects that are not mentioned here, we would like to be notified so that we can report them to the Danish Medicines Agency.

Precautionary measures

Vydura must not be used by pregnant and lactating women.

You must stop taking Vydura for at least 5 days before trying to get pregnant.

Vydura should not be used together with certain specific medicines.

What dose to take?

Emergency treatment: You should take 1 tablet of 75 mg as early as possible in the seizure. Do not take more than 1 tablet of Vydura per day.

Preventive: You should take 1 tablet of 75 mg every other day. If you have a migraine attack on a day when you have already taken Vydura either as a preventative or as a seizure treatment, you may want to use another seizure treatment, such as triptan.

How to take Vydura?

The tablet is placed under the tongue, where it dissolves and is swallowed with the saliva. Do not drink or eat for at least 5 minutes after ingestion.

Subsidies

A clause subsidy is granted to Vydura as a seizure treatment if the following criteria are met:
You need to undergo appropriate preventive treatment.

In addition, you must have tried at least three different triptans, either with no effect or with unacceptable side effects OR have a contraindication to triptans, including having more than 9 treatment-requiring seizures per month.

Supplementation is provided for a maximum of 8 tablets per month.
